Ask your doctor before using chloramphenicol ophthalmic together with ethotoin. Using chloramphenicol ophthalmic with ethotoin can increase the effects of ethotoin. Symptoms of a ethotoin toxicity may include twitching eye movements, slurred speech, drowsiness, loss of balance, tremor, muscle stiffness or weakness, nausea, vomiting, feeling light-headed, fainting, and slow or shallow breathing. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Chloramphenicol often markedly increases serum phenytoin levels. Toxicity has been reported. The mechanism is inhibition of CYP450 metabolism of phenytoin. Phenytoin has also been reported both to increase and to decrease serum chloramphenicol levels by an unknown mechanism. Ethotoin theoretically may behave in a similar fashion. It is not expected that systemic absorption will occur with otic or ophthalmic chloramphenicol. The clinician should, however, be aware of the possibility.

MANAGEMENT: This combination should usually be avoided if possible. If patients do receive these agents concomitantly, serum concentrations should be closely monitored. Patients should be advised to notify their physician if they experience symptoms of hydantoin toxicity, including drowsiness, visual disturbances, change in mental status, seizures, nausea, or ataxia.
