Using glyBURIDE together with fluvastatin can increase the effects of either medication. This can lead to hypoglycemia (low blood sugar). Talk with your doctor before using glyBURIDE while you are using fluvastatin. You may need a dose adjustment and you may need to check your blood sugar more often. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration of fluvastatin and glyburide may result in increased plasma concentrations of both drugs. The proposed mechanism is competitive inhibition of CYP450 2C9 metabolism. In 32 glyburide-treated diabetic patients, administration of fluvastatin (40 mg twice a day for 14 days) increased the mean peak plasma concentration (Cmax), area under the concentration-time curve (AUC) and half-life of glyburide by approximately 50%, 69% and 121%, respectively. Glyburide (5 to 20 mg daily) increased the mean Cmax and AUC of fluvastatin by 44% and 51%, respectively. No changes in glucose, insulin, and C-peptide levels were reported in the study. In 16 healthy volunteers, a lower dosage of fluvastatin (40 mg/day for 15 days) increased the Cmax and AUC of a single 3.5 mg dose of glyburide by approximately 20% only compared to placebo, and there was no change in the hypoglycemic effect of glyburide.

MANAGEMENT: Despite the lack of clinical significance reported in the interaction study, pharmacologic responses to fluvastatin and glyburide should be monitored closely during concomitant use, and dosages of the drugs adjusted accordingly. Patients should be advised to regularly monitor their blood sugar and counseled on how to recognize and treat hypoglycemia, which may include symptoms such as headache, dizziness, drowsiness, nervousness, confusion, tremor, hunger, weakness, perspiration, and palpitations. The physician should be notified if an interaction is suspected.
