Consumer:Dolutegravir may increase the blood levels and effects of metFORMIN. You may need a dose adjustment if you have been receiving metFORMIN and are starting treatment with dolutegravir. Likewise, if you have been receiving both medications, the dose of metFORMIN may need to be adjusted if dolutegravir is discontinued. Contact your doctor if your condition changes or you experience increased side effects. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Coadministration with dolutegravir may increase the plasma concentrations and the risk of adverse effects, including hypoglycemia and lactic acidosis, of metformin. The proposed mechanism is dolutegravir-mediated inhibition of the renal organic cation transporter 2 (OCT2) of which metformin is a substrate. Dolutegravir has been shown in vivo to inhibit tubular secretion of creatinine by inhibiting OCT2.
MANAGEMENT: Caution is advised if dolutegravir must be used concurrently with metformin. The total daily dose of metformin should be limited to 1000 mg if these drugs must be used together. The dose of metformin may require adjustment whenever dolutegravir is added to or withdrawn from therapy. Clinical and laboratory monitoring, including blood glucose monitoring, should be considered when these drugs are used concomitantly.
