A doctor should be consulted to address the issue of joint drug administration.

Consumer:

Atazanavir may increase the blood levels and effects of dolutegravir. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if your condition changes or you experience increased side effects. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with atazanavir may increase the plasma concentrations of dolutegravir. The proposed mechanism is atazanavir inhibition of UGT1A1 and CYP450 3A4, the isoenzymes primarily responsible for the metabolic clearance of dolutegravir. In 12 study subjects, administration of dolutegravir 30 mg once daily with atazanavir 400 mg once daily increased dolutegravir peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin; 24 hours postdose) by 50%, 91% and 180%, respectively, compared to administration without atazanavir. When the same dosage of dolutegravir was given with atazanavir/ritonavir 300 mg/100 mg once daily, the Cmax, AUC and Cmin of dolutegravir increased by 34%, 62% an 121%, respectively. Using cross-study comparisons to historical pharmacokinetic data, dolutegravir did not appear to affect the pharmacokinetics of atazanavir or ritonavir.

MANAGEMENT: Because safety data regarding increased dolutegravir exposures are limited, caution may be advisable if dolutegravir is used in combination with atazanavir, with or without ritonavir.
