Consumer:Using dofetilide together with dolutegravir is not recommended. Combining these medications may increase the blood levels and effects of dofetilide. Your doctor may be able to prescribe alternatives that do not interact. You should seek immediate medical attention if you experience sudden dizziness, lightheadedness, fainting, shortness of breath, or fast or pounding heartbeats during treatment with dofetilide. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:CONTRAINDICATED: Coadministration with dolutegravir may increase the plasma concentrations of dofetilide. The proposed mechanism is dolutegravir inhibition of renal organic cation transporter (OCT2), of which dofetilide is a substrate. Dolutegravir has been shown in vivo to inhibit tubular secretion of creatinine by inhibiting OCT2. Clinically, high plasma levels of dofetilide may increase the risk of QT interval prolongation, which has been associated with ventricular arrhythmias including torsade de pointes and sudden death.
MANAGEMENT: Given the risk for serious and/or life-threatening cardiovascular events associated with increased dofetilide plasma levels, concomitant use with dolutegravir is considered contraindicated.
