Relevance: 22.07.2022 Reviewer: Shkutko P.M., M.D., in

In the database of official manuals used in the service creation an interaction registered by statistical results of studies was found, which can either lead to negative consequences for the patient health or strengthen a mutual positive effect. A doctor should be consulted to address the issue of joint drug administration.

Consumer:

Using fentaNYL together with guanFACINE may slow your heart rate. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or need your blood pressure checked more often to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: A single case of bradycardia has been associated with the combined use of fentanyl (administered via spinal epidural catheter) and orally administered guanfacine. Although it is possible that bradycardia could have been caused by fentanyl in the thoracic vertebral space, this case may represent a drug-drug interaction. The proposed mechanism is combined pharmacologic effect, as fentanyl and guanfacine can each decrease sympathetic outflow to the heart.

MANAGEMENT: While some centrally acting alpha-antagonists, like guanfacine, are used preoperatively, it is recommended that the clinician monitor for bradycardia and be prepared for its consequences.
