Butalbital may reduce the blood levels and effects of dilTIAZem. Contact your doctor if your symptoms worsen or your condition changes during treatment with these medications. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Coadministration with inducers of CYP450 3A4 may significantly decrease the plasma concentrations of diltiazem, which is primarily metabolized by the isoenzyme. In 13 study subjects, administration of diltiazem (240 mg daily for 21 days) with the CYP450 3A4 inducer efavirenz (600 mg daily for 14 days) decreased diltiazem peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) by a mean of 60%, 69% and 63%, respectively. Rifampin, a potent CYP450 3A4 inducer, has been reported to lower diltiazem plasma concentrations to undetectable levels.

MANAGEMENT: Use of diltiazem with rifampin or any known CYP450 3A4 inducer should be avoided when possible, and alternative therapy considered. Diltiazem dose adjustment should be guided by clinical response if concomitant use with a CYP450 3A4 inducer is necessary.
