Duvelisib may increase the blood levels and effects of acetaminophen. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. MONITOR: Coadministration with duvelisib may increase the plasma concentrations of drugs that are primarily metabolized by the CYP450 3A4 isoenzyme. The mechanism is reduced clearance due to inhibition of CYP450 3A4 by duvelisib. The interaction may be particularly important for sensitive CYP450 3A4 substrates or those that demonstrate a narrow therapeutic index. When a single 2 mg oral dose of midazolam, a sensitive CYP450 3A4 substrate, was administered to 14 healthy adult study subjects following multiple doses of duvelisib 25 mg twice daily for 5 days, midazolam peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 2.2- and 4.3-fold, respectively.

MANAGEMENT: Caution is advised if duvelisib is used in combination with sensitive CYP450 3A4 substrates or those that demonstrate a narrow therapeutic index (e.g., ergot alkaloids, colchicine, fentanyl, macrolide immunosuppressants, midazolam, triazolam, vinca alkaloids). Dosage adjustments as well as clinical and laboratory monitoring may be appropriate whenever duvelisib is added to or withdrawn from therapy.
