Activase and Defibrotide Intravenous
Determining the interaction of Activase and Defibrotide Intravenous and the possibility of their joint administration.

Consumer:Using defibrotide together with alteplase is not recommended. Combining these medications may increase the risk of bleeding. If possible, it is best to wait until the effects of one drug has worn off before starting the other. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:CONTRAINDICATED: Coadministration of defibrotide with fibrinolytic therapy may increase the risk of bleeding. In vitro, defibrotide increased the activity of fibrinolytic enzymes. In a mouse model of thromboembolism, recombinant tissue plasminogen activator (t-PA) potentiated the antithrombotic effect of defibrotide when given intravenously.
MANAGEMENT: Concomitant use of defibrotide with fibrinolytic therapy is considered contraindicated. Fibrinolytic agents should be discontinued prior to initiation of defibrotide. Preferably, defibrotide administration should be delayed until the effects of fibrinolytic therapy have subsided. Patients should be monitored for signs of bleeding during defibrotide therapy. If bleeding develops, defibrotide should be discontinued and treatment/supportive care provided until the bleeding stops.
