Colesevelam may increase the blood levels and effects of metFORMIN. You may need a dose adjustment or closer monitoring of your blood sugar to safely use both medications. Contact your doctor if your condition changes or you experience increased side effects such as nausea, vomiting, diarrhea, flatulence, and indigestion. You should seek immediate medical attention if you develop weakness, muscle pain, chills, increasing drowsiness, abdominal pain or discomfort, slow or irregular heartbeat, breathing difficulty and other unusual symptoms, as these may be early signs of a rare but serious side effect of metFORMIN known as lactic acidosis. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with colesevelam may increase the plasma concentrations of metformin. The mechanism of interaction has not been described. When 1500 mg extended-release metformin was administered in combination with 3.75 grams of colesevelam, metformin peak plasma concentration (Cmax) increased by 8% and systemic exposure (AUC) increased by 44%.

MANAGEMENT: Caution is advised if metformin is used concomitantly with colesevelam. Clinical response and tolerance to metformin should be monitored more closely following the initiation or discontinuation of colesevelam. Patients should be advised to contact their physician if they experience increased side effects of metformin such as nausea, vomiting, diarrhea, flatulence, indigestion, and asthenia. Immediate medical attention should be sought if they experience potential signs and symptoms of lactic acidosis such as malaise, myalgia, somnolence, abdominal distress, respiratory distress, hyperventilation, slow or irregular heartbeat, and hypothermia.
