Before taking pioglitazone, tell your doctor if you also use pregabalin. Taking these medications may cause weight gain and fluid retention which could lead to heart failure. If you take both medications together, tell your doctor if you have any of these symptoms. You may need a dose adjustment if you take both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Higher frequencies of weight gain and peripheral edema were observed in patients taking both pregabalin and a thiazolidinedione antidiabetic agent compared to patients taking either drug alone, presumably due to additive effects during coadministration. In the overall safety database for pregabalin, peripheral edema and weight gain were reported in 3% (2/60) and 0% (0/60), respectively, of patients who were using thiazolidinedione antidiabetic agents only; 8% (69/859) and 4% (35/859), respectively, of patients who were treated with pregabalin only; and 19% (23/120) and 7.5% (9/120), respectively, of patients who were on both pregabalin and thiazolidinedione antidiabetic agents. The majority of these patients were participants in studies of pain associated with diabetic peripheral neuropathy.

MANAGEMENT: Since fluid retention and weight gain may exacerbate or lead to heart failure in susceptible patients, caution is advised if pregabalin is used in combination with a thiazolidinedione. Because there are limited data on congestive heart failure patients with New York Heart Association (NYHA) Class III or IV cardiac status, pregabalin with or without a thiazolidinedione should be used cautiously in such patients.
