Ranolazine may increase the blood levels and effects of metFORMIN. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Do not stop using any medications without first talking to your doctor.

MONITOR: Ranolazine may reduce the renal clearance of drugs that are substrates of the organic cation transporter 2 (OCT2). The mechanism is inhibition of OCT2-mediated elimination by ranolazine. During concomitant use of metformin 1000 mg twice a day with ranolazine 500 mg to 1000 mg twice a day in subjects with type 2 diabetes, the plasma exposure to metformin increased by 1.4-fold and 1.8-fold, respectively.

MANAGEMENT: It may be advisable to monitor patients for altered efficacy and safety if ranolazine must be used with an OCT2 substrate.
