The mechanism for this interaction has not been delineated. In a clinical study, topiramate decreased the AUC of pioglitazone by 15%, but did not alter Cmax. In addition, topiramate reduced the Cmax and AUC of the active hydroxy-metabolite by 13% and 16%, respectively, as well as a 60% decrease in Cmax and AUC of the active keto-metabolite. The clinical significance of these findings is unknown. In another study, Cmax and AUC of glyburide were reduced by 22% and 25%, respectively, during concomitant topiramate administration. In addition, the AUC of the two active metabolites were also reduced by 13% and 15%, and Cmax was reduced by 18% and 25%, respectively. MANAGEMENT: Although these changes in exposure to pioglitazone and glyburide may not be clinically important, careful monitoring of glycemic control is recommended during concomitant therapy with topiramate.
