Consumer:Using meperidine together with acyclovir may increase the effects of meperidine. Contact your doctor if you experience extreme drowsiness, muscle weakness, confusion, cold and clammy skin, pinpoint pupils, shallow breathing, slow heart rate, fainting, or coma. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Acyclovir may increase the plasma concentrations of meperidine and normeperidine, the metabolite that has been associated with adverse effects such as lethargy, agitation, and seizures. The mechanism of interaction is unknown. Increased pharmacologic and adverse effects of meperidine may occur.
MANAGEMENT: Caution is recommended if meperidine is used with acyclovir or its prodrug, valacyclovir. Patients should be monitored for tremor, seizures, CNS and respiratory depression, and cardiac arrhythmias.
