NIFEdipine may increase the blood levels of flibanserin. MONITOR: Coadministration with weak inhibitors of CYP450 3A4 may increase the plasma concentrations of flibanserin, which is primarily metabolized by CYP450 3A4 and, to a lesser extent, by CYP450 2C19. In a meta-analysis of 17 oral contraceptive users and 91 non-users in Phase 1 studies, flibanserin peak plasma concentration (Cmax) and systemic exposure (AUC) were 30% and 40% higher, respectively, in oral contraceptive users than in non-users. Ethinyl estradiol, specifically, is a known weak inhibitor of CYP450 3A4 and 2C19, although other ingredients in oral contraceptives such as progestins are known substrates of CYP450 3A4 and may also be weak inhibitors. The clinical significance of this interaction is unknown. However, multiple weak CYP450 3A4 inhibitors could conceivably produce clinically relevant increases in flibanserin concentrations that may potentiate the risk of hypotension, syncope, and central nervous system depression.

MANAGEMENT: Caution is advised when flibanserin is used with multiple weak CYP450 3A4 inhibitors. Patients should be monitored for signs and symptoms of hypotension, syncope, and central nervous system depression.
