Sucroferric oxyhydroxide and tetracycline should not be taken orally at the same time. Products that contain iron such as sucroferric oxyhydroxide may interfere with the absorption of tetracycline and reduce its effectiveness. To prevent or minimize the interaction, you should take tetracycline at least one to four hours before sucroferric oxyhydroxide. ADJUST DOSING INTERVAL: Coadministration with sucroferric oxyhydroxide may reduce the oral bioavailability of tetracyclines. The proposed mechanism is chelation of tetracyclines to the ferric ion component of sucroferric oxyhydroxide, forming an insoluble complex that is poorly absorbed from the gastrointestinal tract. According to the product labeling for sucroferric oxyhydroxide, an in vitro interaction with doxycycline was observed when studied in aqueous solutions that mimic the physico-chemical conditions of the gastrointestinal tract. This is consistent with previous pharmacokinetic studies that have demonstrated a substantial interaction between tetracyclines and iron salts. In ten healthy volunteers, simultaneous oral administration of ferrous sulfate 200 mg and single doses of various tetracyclines (200 mg to 500 mg) resulted in reductions in the serum levels of methacycline and doxycycline by 80% to 90%, oxytetracycline by 50% to 60%, and tetracycline by 40% to 50%. In another study, 300 mg of ferrous sulfate reduced the absorption of tetracycline by 81% and that of minocycline by 77%.

MANAGEMENT: Doxycycline should be administered at least 1 hour before sucroferric oxyhydroxide. Omadacycline administration with sucroferric oxyhydroxide should be separated by 4 hours. No dosing recommendations are available for use with other tetracyclines, although patients should consider following the same precaution.
