Consumer:Using dilTIAZem and alfuzosin together can lower your blood pressure and slow your heart rate. This can cause a slow heartbeat, headaches, dizziness, or feeling like you might pass out. You may need a dose adjustment or need your blood pressure checked more often if you take both medications. Professional:MONITOR: Concomitant administration of diltiazem may increase the plasma concentration of alfuzosin. The mechanism is inhibition of CYP450 3A4 metabolism by diltiazem. Multiple doses of diltiazem 240 mg with alfuzosin 2.5 mg three times daily increased alfuzosin maximum plasma concentrations (Cmax) by 1.5-fold and the area under the concentration-time curve (AUC) 1.3-fold. In addition, the diltiazem Cmax increased 1.4-fold; the mechanism has not been reported. No changes in blood pressure were reported in this study; however, the manufacturer cautions that additive hypotension may occur when alfuzosin is given in combination with antihypertensive agents.
MANAGEMENT: Hemodynamic responses should be monitored during coadministration, especially during the first few weeks of therapy. Patients should be advised to avoid situations where injury could result if syncope occurs (e.g., driving) and to notify their physician if they experience dizziness or syncope.
