The risk of aluminum toxicity may be further increased during treatment with vitamin D or vitamin D analogs like calcitriol, which increase the absorption of aluminum from the intestine. You should avoid taking medications that contain aluminum if you have impaired kidney function unless otherwise directed by your doctor. Always check the labels of over-the-counter medications to be sure they do not contain aluminum before you use them. Talk to your doctor or other healthcare professional if you are not sure whether a product contains aluminum. They may also be able to suggest alternative medications when necessary. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: In patients with advanced renal impairment or on dialysis, chronic use of aluminum-containing preparations during treatment with vitamin D analogs may result in increased blood levels of aluminum and aluminum toxicity. Patients with renal failure are at risk for toxicity due to impaired clearance of aluminum, and the risk may be further increased by the administration of vitamin D analogs due to increased intestinal absorption of aluminum. Toxicity may manifest as osteodystrophy, osteomalacia, myopathy, anemia, and encephalopathy.
MANAGEMENT: Concomitant use of vitamin D analogs and aluminum-containing preparations should be followed with monitoring for signs and symptoms of aluminum toxicity.
