Consumer:

Using cyclophosphamide together with allopurinol may increase the effects of cyclophosphamide. Contact your doctor if you experience decreased bone marrow function and blood problems (extreme fatigue, easy bruising or bleeding; black, bloody or tarry stools, fever or chills, or signs of infection such as fever, chills, or sore throat); bladder problems; and others. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Allopurinol may enhance the myelosuppressive side effects of cyclophosphamide. Allopurinol does not appear to increase the serum half-life of cyclophosphamide, but has been shown to cause a 38% average increase in the plasma concentration of cyclophosphamide metabolites. It is suspected that the mechanism is either induction of hepatic metabolism of the parent cyclophosphamide compound or decreased renal clearance of cyclophosphamide metabolites. Clinically, this interaction could increase the risk of bleeding and infection.

MANAGEMENT: If these drugs must be given together, frequent monitoring for myelosuppression, hemorrhagic cystitis, cardiotoxicity, interstitial pneumonitis, and bleeding is recommended.
