Coadministration with tolvaptan may increase the plasma concentrations of lovastatin and its active metabolite, lovastatin beta-hydroxyacid. The mechanism is tolvaptan inhibition of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of lovastatin as well as the metabolite. According to the tolvaptan product labeling, administration of lovastatin in combination with tolvaptan increased the systemic exposure to lovastatin and lovastatin beta-hydroxyacid by 1.4- and 1.3-fold, respectively. These changes are not considered clinically significant.
