Consumer:Aluminum hydroxide may decrease the absorption and blood levels of elvitegravir when taken at the same time. Therefore, aluminum hydroxide should be taken at least 2 hours before or 2 hours after you take elvitegravir. Consult with your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:ADJUST DOSING INTERVAL: The concomitant use of antacids may significantly decrease the plasma concentrations of elvitegravir. The mechanism of interaction has not been described. When 20 mL of antacid was given 2 hours before a 50 mg dose of elvitegravir with ritonavir (100 mg pharmacokinetic booster dose) to 11 study subjects, mean elvitegravir peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) were reduced by 18%, 15% and 10%, respectively. When the antacid was given 2 hours after elvitegravir to 10 study subjects, mean elvitegravir Cmax, AUC, and Cmin decreased by approximately 20% each. Larger decreases may be expected if the antacid and elvitegravir doses were administered closer together, although data have not been reported. In contrast, when the antacid was given 4 hours before or 4 hours after elvitegravir, no significant differences were observed in elvitegravir pharmacokinetics compared to administration of elvitegravir alone.
MANAGEMENT: Antacid-containing preparations should be administered at least 2 hours before or 2 hours after the elvitegravir dose. Alternatively, H2-receptor antagonists or proton pump inhibitors may be considered, with no dosage adjustment needed.
