Consumer:

CycloSPORINE may significantly increase the blood levels of ambrisentan. This can increase the risk of serious side effects such as liver problems, anemia, and fluid accumulation (particularly in the lungs). You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor immediately if you experience fever, chills, joint pain or swelling, unusual bleeding or bruising, skin rash, itching, loss of appetite, fatigue, nausea, vomiting, dark colored urine, and/or yellowing of the skin or eyes, as these may be signs and symptoms of liver damage. Also seek medical attention if you develop paleness, dizziness, fainting, swelling, unusual weight gain, difficulty breathing, and/or shortness of breath. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSE: Coadministration with cyclosporine may increase the plasma concentrations of ambrisentan. The mechanism is cyclosporine inhibition of P-glycoprotein efflux transporter and organic anion transporting polypeptide, of which ambrisentan is a substrate. In healthy volunteers, administration of ambrisentan (5 mg once daily) in combination with cyclosporine (twice daily regimen targeting a trough concentration of 150 to 200 ng/mL) for 14 days resulted in an approximately 1.5-fold increase in ambrisentan peak plasma concentration (Cmax) and a 2-fold increase in systemic exposure (AUC). Ambrisentan had no significant effect on the pharmacokinetics of cyclosporine.

MANAGEMENT: The dosage of ambrisentan should be limited to 5 mg once daily when coadministered with cyclosporine. Patients should be monitored for adverse effects such as anemia, fluid retention, pulmonary edema, and hepatotoxicity.
