Using midazolam together with atorvastatin may increase the effects of midazolam. Contact your doctor if you experience extreme drowsiness, confusion, muscle weakness, slurred speech, tremors, a slow heartbeat, shallow breathing, feeling light-headed, fainting, or seizures. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with atorvastatin may increase the plasma concentrations of midazolam. The proposed mechanism is competitive inhibition of midazolam metabolism via CYP450 3A4, of which atorvastatin is also a substrate. In a study involving patients who were receiving intravenous midazolam (0.15 mg/kg) as an adjunct for general anesthesia, seven patients on long-term atorvastatin therapy (10 to 40 mg/day) demonstrated, on average, a 41% greater area under the plasma concentration-time curve (AUC) for midazolam than seven matched control patients. Clearance was 33% lower in the atorvastatin group compared to the control group. The clinical significance of this interaction is unknown, although an increased risk of prolonged sedation and respiratory depression is conceivable.

MANAGEMENT: In patients receiving chronic atorvastatin, it may be prudent to consider the potential for respiratory depression and prolonged sedation if midazolam is used in high dosages as the main or sole hypnotic agent.
