Using cycloSPORINE together with sulfinpyrazone may decrease the effects of cycloSPORINE. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: The coadministration with sulfinpyrazone may decrease the blood concentrations and pharmacologic effects of cyclosporine. The mechanism is unknown. In an uncontrolled retrospective study of 120 heart transplant recipients receiving sulfinpyrazone 200 mg daily for 4 to 8 months, trough cyclosporine levels decreased by an average of 39%, even through the daily dose of cyclosporine increased by 7.7%. Two cases of acute rejection occurred; one after 5 years and one after 7 years post-transplant, during concomitant treatment. Cyclosporine levels had decreased to 50 mcg/L and 20 mcg/L, respectively. Both patients were treated successfully with intravenous steroids.

MANAGEMENT: Given the risk of organ rejection associated with inadequate immunosuppressant levels, caution is advised if cyclosporine must be coadministered with sulfinpyrazone. Cyclosporine blood levels should be monitored regularly and the dosage adjusted accordingly, particularly following initiation or discontinuation of sulfinpyrazone therapy in patients who are stabilized on their anti-rejection regimen.
