The proposed mechanism is increased clearance due to apalutamide-mediated induction of CYP450 3A4, which is the isoenzyme primarily responsible for the metabolism of ritonavir. In addition, concomitant use of ritonavir may increase the plasma levels of apalutamide and risk of adverse effects, including seizures and fracture. The proposed mechanism is ritonavir-mediated inhibition of CYP450 3A4. MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, concomitant use of ritonavir with apalutamide is considered contraindicated.
