Eslicarbazepine may reduce the blood levels of warfarin in some patients. This may make the medication less effective in treating and preventing blood clots. You should seek immediate medical attention if you experience potential signs and symptoms of blood clots such as chest pain, shortness of breath, difficulty breathing, coughing up blood, sudden loss of vision, and/or pain, redness or swelling in an arm or leg. You may need a dose adjustment or more frequent monitoring of your INR to safely use both medications, especially during the first 2 to 3 weeks after starting or stopping eslicarbazepine. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with eslicarbazepine acetate may decrease the plasma concentrations of S(-) warfarin, the more pharmacologically active of the two enantiomers of warfarin. The mechanism of interaction has not been established. In vitro studies have shown that eslicarbazepine does not significantly affect the plasma protein binding of warfarin. In healthy study subjects, S(-) warfarin systemic exposure (AUC) decreased modestly by an average of 23% when eslicarbazepine acetate 1200 mg once daily was coadministered with warfarin. The pharmacokinetics of R(+) warfarin and coagulation parameters were not affected.

MANAGEMENT: Given the potential for interaction and the high degree of interpatient variability with respect to warfarin metabolism, patients receiving warfarin should have INR monitored closely during the first 2 to 3 weeks following initiation or discontinuation of eslicarbazepine acetate. The same precaution may be applicable during therapy with other similar anticoagulants, although clinical data are lacking. Patients should be advised to seek immediate medical attention if they experience potential signs and symptoms of blood clots such as chest pain, shortness of breath, sudden loss of vision, and pain, redness or swelling in an extremity.
