Coadministration with tipranavir/ritonavir may reduce the plasma concentrations of raltegravir. The proposed mechanism is induction of raltegravir glucuronidation via uridine diphosphate glucuronosyltransferase (UGT) 1A1. In 15 study subjects, administration of raltegravir 400 mg twice daily in combination with tipranavir/ritonavir 500 mg/200 mg twice daily decreased the raltegravir peak plasma concentration (Cmax), systemic exposure (AUC), and trough plasma concentration (Cmin) by 18%, 24%, and 55%, respectively. The long-term clinical significance of these changes, particularly the reduction in Cmin, is uncertain. In two clinical trials, comparable efficacy was observed in approximately 100 subjects who received raltegravir in combination with tipranavir/ritonavir relative to subjects who did not receive tipranavir/ritonavir. Based on these data, no dosage adjustment of raltegravir is necessary when used together with tipranavir/ritonavir.
