Using fluvoxaMINE together with glimepiride can increase the effects of glimepiride. You should be advised to regularly monitor your blood sugar and counseled on how to recognize and treat hypoglycemia (low blood sugar), which may include symptoms such as headache, dizziness, drowsiness, nervousness, confusion, tremor, hunger, weakness, perspiration, and palpitations. You may need a dose adjustment or blood sugar checked more often to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: The coadministration with fluvoxamine may increase the plasma concentrations of glimepiride. The mechanism probably involves increased absorption of glimepiride and/or decreased clearance due to inhibition of the CYP450 2C9 enzymatic pathway by fluvoxamine. In a randomized, placebo-controlled, crossover study consisting of 12 healthy subjects, fluvoxamine (100 mg orally daily for 4 days) increased the peak plasma concentration (Cmax) of a single dose of glimepiride (0.5 mg) by 43%. The elimination half-life was prolonged from 2 to 2.3 hours, while the area under the concentration-time curve (AUC) was not significantly affected. Fluvoxamine also did not alter the effects of glimepiride on blood glucose levels.

MANAGEMENT: As a precaution, it may be appropriate to monitor blood glucose more closely during the concomitant use of glimepiride and fluvoxamine, and adjust the glimepiride dosage if necessary. Patients should be counseled to recognize the signs of hypoglycemia (e.g., headache, dizziness, drowsiness, nausea, tremor, weakness, or palpitations) and how to treat it.
