Using clarithromycin together with delavirdine may increase the effects of clarithromycin. Contact your doctor if you experience increased side effects or if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

ADJUST DOSE: Concomitant administration of delavirdine and clarithromycin has been shown to increase the area under the plasma concentration-time curve (AUC) of clarithromycin by 100%. The mechanism may be related to inhibition of CYP450 3A4 hepatic metabolism.

MANAGEMENT: Close clinical monitoring for toxicity is recommended if this combination is used. The manufacturer recommends dose adjustments for clarithromycin in patients with renal insufficiency: Reduce the dose by 50% for patients with a creatinine clearance of 30 to 60 mL/min, and reduce by 75% for patients with creatinine clearance less than 30 mL/min.
