Biktarvy and Dofetilide

Consumer:Using dofetilide together with bictegravir is not recommended. Combining these medications may increase the blood levels and effects of dofetilide. This can increase the risk of an irregular heart rhythm that may be serious and potentially life-threatening. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact. You should seek immediate medical attention if you experience sudden dizziness, lightheadedness, fainting, shortness of breath, or fast or pounding heartbeats during treatment with dofetilide. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:CONTRAINDICATED: Coadministration with bictegravir may increase the plasma concentrations of dofetilide. The proposed mechanism is inhibition of organic cation transporter 2 (OCT2) and multidrug and toxin extrusion transporter 1 (MATE1) by bictegravir, resulting in decreased renal tubular secretion of dofetilide. Clinically, high plasma levels of dofetilide may increase the risk of QT interval prolongation, which has been associated with ventricular arrhythmias including torsade de pointes and sudden death.
MANAGEMENT: Given the risk for serious and/or life-threatening cardiovascular events associated with increased dofetilide plasma levels, concomitant use with bictegravir is considered contraindicated.
