Consumer:Using phenytoin together with praziquantel may decrease the effects of praziquantel. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:ADJUST DOSE: One study has demonstrated that the oral bioavailability of praziquantel decreases significantly when it is given to patients maintained on phenytoin. Maximum plasma praziquantel concentrations in patients receiving phenytoin were 24% of those in control subjects. The proposed mechanism is induction of CYP450 hepatic metabolism. Treatment failure may occur.
MANAGEMENT: Increasing the praziquantel dose or adding cimetidine to the regimen (which is expected to increase praziquantel serum levels) have been suggested as means of controlling the interaction; however, their effectiveness has not been definitely established. Clinical response should be closely monitored.
