Consumer:Ask your doctor before using bretylium together with digoxin. Bretylium has been found to increase the effects of digoxin. This can cause nausea, vomiting, seizures, hallucinations, light "halos" around objects, green or yellow vision, fatigue, irregular heartbeats, and abnormally fast or slow heartbeats. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:GENERALLY AVOID: Administration of bretylium may exacerbate digoxin toxicity and digoxin-induced arrhythmias. The mechanism is related to initial norepinephrine release induced by bretylium.
MANAGEMENT: Bretylium should be used cautiously in patients on digoxin and should be avoided if digitalis toxicity is present. Simultaneous initiation of therapy should also be avoided.
