Based on both in vitro and in vivo studies, paroxetine given at normally recommended dosages does not appear to significantly affect the pharmacokinetics or cardiovascular effects of terfenadine. In 12 healthy male volunteers, administration of terfenadine (60 mg twice daily for 8 days) with paroxetine (20 mg once daily for 15 days) did not alter steady-state pharmacokinetics of terfenadine or rate-corrected QT interval (QTc) compared to administration of terfenadine alone. There was a small, unexplained reduction (on average 17% to 20%) in the steady-state peak plasma concentration (Cmax) and systemic exposure(AUC) of the metabolite, carboxyterfenadine, during coadministration with paroxetine; however, this alteration is unlikely to be of clinical significance. Caution may be warranted when high dosages of paroxetine is administered.
