Professional:ADJUST DOSE: Coadministration with rifampin may significantly decrease the plasma concentrations of brivaracetam. The proposed mechanism is rifampin induction of the CYP450 2C19-mediated metabolism of brivaracetam. In healthy study subjects, administration of brivaracetam with rifampin (600 mg/day for 5 days) resulted in a 45% decrease in brivaracetam systemic exposure (AUC) compared to administration alone.
MANAGEMENT: The manufacturer recommends increasing the brivaracetam dosage by up to 100% (i.e., doubling the dosage) when used with rifampin. Clinical response and tolerability should be monitored closely.
