MONITOR: Coadministration with somatostatin analogs may increase the plasma concentrations of bromocriptine. The mechanism may involve inhibition of CYP450 3A4 metabolism secondary to suppression of growth hormones by somatostatin analogs. In a study of 12 patients with active acromegaly, the bioavailability of bromocriptine was increased by approximately 40% during coadministration with octreotide relative to administration alone. No data are available for lanreotide or pasireotide, although a similar interaction is theoretically possible.

MANAGEMENT: Caution is advised when bromocriptine is prescribed in combination with somatostatin analogs. Monitoring for increased adverse effects is recommended, including: nausea; vomiting; dizziness; hypotension; somnolence or sudden sleep onset, particularly in patients with Parkinson's disease; and cold-sensitive digital vasospasm in acromegalic patients.
