Clopidogrel may increase the blood levels and effects of buPROPion. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if you experience seizures, anxiety, tremor, insomnia, hallucinations, confusion, and/or difficulty concentrating, as these may be signs and symptoms of excessive buPROPion levels. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with clopidogrel may increase the plasma concentrations of bupropion. The proposed mechanism is clopidogrel inhibition of bupropion hydroxylation via CYP450 2B6. In 12 healthy male volunteers, administration of a single 150 mg dose of bupropion in combination with clopidogrel (75 mg orally once daily for 4 days) resulted in a 40% increase in bupropion peak plasma concentration (Cmax) and 60% increase in systemic exposure (AUC) compared to administration of bupropion alone. Additionally, the AUC of the hydroxybupropion metabolite decreased by 52% and the AUC ratio of hydroxybupropion to bupropion decreased by 68% with clopidogrel, while oral clearance decreased by 26%.

MANAGEMENT: Caution is advised if bupropion is prescribed with clopidogrel. Pharmacologic response to bupropion should be monitored more closely whenever clopidogrel is added to or withdrawn from therapy, and the bupropion dosage adjusted as necessary. Patients should be advised to contact their physician if they experience potential symptoms of bupropion toxicity such as agitation, anxiety, tremor, insomnia and seizures, or neuropsychiatric symptoms such as delusions, hallucinations, psychosis, impaired concentration, paranoia and confusion.
