Professional:GENERALLY AVOID: Coadministration with iron chelating agents, such as deferasirox, may result in an increase in the plasma concentrations and the risk of toxicity of busulfan. A reduction in the clearance of busulfan is suspected via an undetermined mechanism.
MANAGEMENT: Discontinuation of iron chelating agents prior to initiating treatment with busulfan is recommended. Deferasirox should be discontinued approximately 5 half-lives (2 to 3 days) before initiating busulfan. If concomitant therapy is unavoidable, a dosage reduction as well as clinical and laboratory monitoring of busulfan should be considered.
