Busulfan Tablet and Deferasirox

Using deferasirox together with busulfan may increase the blood levels and effects of busulfan. Treatment with deferasirox may need to be stopped several days before beginning treatment with busulfan. GENERALLY AVOID: Coadministration with iron chelating agents, such as deferasirox, may result in an increase in the plasma concentrations and the risk of toxicity of busulfan. A reduction in the clearance of busulfan is suspected via an undetermined mechanism.

MANAGEMENT: Discontinuation of iron chelating agents prior to initiating treatment with busulfan is recommended. Deferasirox should be discontinued approximately 5 half-lives (2 to 3 days) before initiating busulfan. If concomitant therapy is unavoidable, a dosage reduction as well as clinical and laboratory monitoring of busulfan should be considered.
