Consumer:

Canagliflozin may cause salt and water loss, which may increase the risk of dehydration and low blood pressure when used with moexipril or similar medications. You may experience headache, dizziness, lightheadedness, fainting, and/or changes in pulse or heart rate. Use caution when getting up from a sitting or lying position, and let your doctor know if you develop these symptoms. In addition, combining these medications may increase the risk of hyperkalemia, or high blood potassium. In severe cases, hyperkalemia can lead to kidney failure, muscle paralysis, irregular heart rhythm, and cardiac arrest. You may be more likely to develop hyperkalemia while taking these medications if you are elderly or have kidney disease or advanced heart failure. Regular or long-term use of nonsteroidal anti-inflammatory drugs such as ibuprofen or naproxen (Aleve) may also increase your risk. Talk to your doctor to see if you should limit consumption of potassium-rich foods such as tomatoes, raisins, figs, potatoes, lima beans, bananas, plantains, papayas, pears, cantaloupes, mangoes, and potassium-containing salt substitutes. You should seek medical attention if you experience nausea, vomiting, weakness, confusion, tingling of the hands and feet, a weak pulse, or a slow or irregular heartbeat, as these may be symptoms of hyperkalemia. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Canagliflozin may potentiate the hypotensive and hyperkalemic effects of drugs that interfere with the renin-angiotensin-aldosterone system. Inhibition of glucose and sodium co-transport by canagliflozin produces mild diuresis and transient natriuresis, resulting in intravascular volume contraction. Symptomatic hypotension and hyperkalemia can occur after initiating treatment with canagliflozin, and the risk may be increased with concomitant use of agents like angiotensin-converting-enzyme (ACE) inhibitors, angiotensin receptor blockers (ARBs), and renin inhibitors.

MANAGEMENT: Caution is advised if canagliflozin is coadministered with ACE inhibitors, ARBs or renin inhibitors, particularly in the elderly and patients with impaired renal function (eGFR less than 60 mL/min/1.73 m2). Prior to initiating canagliflozin, volume status should be assessed and corrected, if necessary. Clinical and laboratory monitoring are recommended during therapy, including electrolytes, fluid status, renal function, and blood pressure. Patients should be given dietary counseling to avoid excessive intake of potassium-rich foods and salt substitutes, and advised to seek medical attention if they experience signs and symptoms of hyperkalemia such as nausea, vomiting, weakness, listlessness, tingling of the extremities, paralysis, confusion, weak pulse, and a slow or irregular heartbeat.
