CarBAMazepine may reduce the blood levels and effects of ezogabine. You may need a dose adjustment if you have been taking ezogabine and are starting treatment with carBAMazepine. ADJUST DOSE: Coadministration with carbamazepine or phenytoin may decrease the plasma concentrations of ezogabine. The mechanism may involve induction of ezogabine metabolism via uridine 5'-diphosphate-glucuronyltransferases (UGTs). In a phase 2 study, administration of ezogabine 300 to 1200 mg/day in combination with carbamazepine 600 to 2400 mg/day reduced the ezogabine peak plasma concentration (Cmax) and systemic exposure (AUC) by 23% and 31%, respectively, and increased the clearance by 28%. Administration of ezogabine with phenytoin 120 to 600 mg/day reduced the ezogabine Cmax and AUC by 18% and 34%, respectively, and increased the clearance by 33%.

MANAGEMENT: An increase in dosage of ezogabine should be considered when adding phenytoin or carbamazepine. Likewise, the ezogabine dosage may need to be reduced when these agents are discontinued.
