Carfilzomib may increase the blood levels and effects of digoxin. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with carfilzomib may increase the plasma concentrations of drugs that are substrates of P-glycoprotein (P-gp), such as digoxin and colchicine. The proposed mechanism is carfilzomib inhibition of P-gp-mediated efflux of these drugs. In vitro, carfilzomib has been shown to inhibit the efflux transport of digoxin by 25% at concentrations lower than those expected at therapeutic doses.

MANAGEMENT: Caution is advised if carfilzomib is used concomitantly with drugs that are substrates of the P-gp transport protein, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring should be considered whenever carfilzomib is added to or withdrawn from therapy with these drugs. In patients receiving digoxin, serum digoxin levels should be monitored more closely and patients should be advised to contact their doctor if they experience signs and symptoms of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeat. For those receiving colchicine, patients should be advised to contact their physician if they experience symptoms of toxicity such as abdominal pain, nausea, vomiting, diarrhea, fatigue, myalgia, asthenia, hyporeflexia, paraesthesia, and numbness.
