Using spironolactone together with cholestyramine may alter the effects of spironolactone. Contact your doctor if you experience numbness or tingly feeling, muscle pain or weakness, slow, fast, or uneven heartbeat, feeling drowsy, restless, or light-headed, urinating less than usual, shallow breathing, tremors, confusion, or nausea. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Rare cases of severe hyperchloremic, hyperkalemic, metabolic acidosis have been associated with coadministration of spironolactone and cholestyramine. While spironolactone alone can cause metabolic acidosis, the addition of cholestyramine could provide an anion exchange of chloride for bicarbonate in the small bowel. The resulting loss of bicarbonate predisposes the patient to a hyperchloremic, nonanion gap, metabolic acidosis in the presence of renal failure.

MANAGEMENT: If these drugs must be used together, careful monitoring of electrolytes and metabolic status is strongly recommended, particularly in the presence of renal insufficiency.
