Consumer:Using carBAMazepine together with cimetidine may increase the effects of carBAMazepine. Contact your doctor if you experience shallow breathing, fast heartbeat, nausea, vomiting, urinating less or not at all, muscle twitches, restlessness, tremors, slurred speech, staggering walk, and feeling light-headed or fainting. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Cimetidine may increase carbamazepine levels by inhibiting its CYP450 3A4 hepatic metabolism. Carbamazepine toxicity is possible.
MANAGEMENT: If these drugs must be used together, close observation for clinical and laboratory evidence of carbamazepine toxicity is recommended. Patients should be advised to report possible symptoms of carbamazepine toxicity (nausea, visual disturbances, dizziness, or ataxia). Dose reductions may be required. Famotidine, nizatidine and ranitidine are not expected to inhibit hepatic metabolism and may be considered as alternatives.
