Cimetidine has been reported to increase the area under the concentration time curve of orally administered azelastine by 65%. The mechanism of the interaction is unknown, but may be related to inhibition by cimetidine of hepatic enzymes responsible for the metabolism of azelastine. The importance of this interaction with intranasal or ophthalmic azelastine is unknown. Patients requiring this combination should be observed for excessive azelastine adverse effects. Ranitidine has not been shown to affect the pharmacokinetic disposition of azelastine and may provide an alternative to cimetidine.
