Using clopidogrel together with rifAMPin may increase the effects of clopidogrel and cause you to bleed more easily. Call your doctor promptly if you have any unusual bleeding or bruising, vomiting, blood in your urine or stools, headache, dizziness, or weakness. You may need a dose adjustment in addition to tests to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with rifampin may increase the metabolic activation of the prodrug clopidogrel, resulting in enhanced antiplatelet effects. The proposed mechanism is rifampin induction of CYP450 3A4, the isoenzyme responsible for the conversion of clopidogrel to its pharmacologically active form. In ten healthy volunteers, platelet aggregation was 33% following coadministration of rifampin 300 mg twice a day and clopidogrel 75 mg/day for 6 days, compared to 56% following clopidogrel alone for 6 days and 94% at baseline. The clinical significance of these changes are unknown.

MANAGEMENT: Until more information is available, monitoring for altered efficacy of clopidogrel may be advisable if rifampin is added to or discontinued from a regimen. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
