Using cloZAPine together with FLUoxetine may increase the effects of cloZAPine. Contact your doctor if you experience confusion, fast heart rate, drowsiness, drooling, weak or shallow breathing, feeling like you might pass out, and seizure (convulsions). If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Several studies have suggested that fluoxetine may increase serum clozapine levels significantly. In one study, clozapine levels were increased by 76% and norclozapine levels were increased by 52%. The mechanism of this interaction is unknown but may be related to inhibition of CYP450 hepatic metabolism by fluoxetine. No adverse effects were reported in a small study; however, myoclonus was reported in one case report.

MANAGEMENT: Close clinical observation is recommended if clozapine and fluoxetine must be used together. The dose of clozapine may require reduction if an interaction is suspected.
