Consumer:

Artemether may reduce the blood levels and effects of norethindrone. If you are using birth control pills or another hormone-type contraceptive such as a patch, shot, vaginal ring or implant, you should talk to your doctor before taking artemether. You may need an alternative or additional method of birth control during treatment with artemether in order to avoid an unintended pregnancy. Let your doctor know if you experience bleeding outside of your menstrual cycle, since it may indicate reduced effectiveness of norethindrone. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADDITIONAL CONTRACEPTION RECOMMENDED: Coadministration with artemether may decrease the plasma concentrations of contraceptive hormones. The proposed mechanism is artemether induction of CYP450 3A4, the isoenzyme primarily responsible for the metabolic clearance of sex hormones. Clinical data are lacking.

MANAGEMENT: Women using hormonal contraceptives should be advised of the risk of breakthrough bleeding and unintended pregnancy during concomitant therapy with artemether-lumefantrine. Alternative or additional methods of birth control should be used. Intrauterine systems are unlikely to be significantly affected because of their local action. Input from a gynecologist or similar expert on adequate contraception, including emergency contraception, should be sought as needed.
