Using colchicine together with digoxin may elevate the blood levels of both drugs, increasing the risk of serious side effects that can affect your muscles, blood cells, nervous system, and multiple organs including the liver and kidneys. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Let your doctor know if you experience irregular heartbeats, vision abnormalities, abdominal pain, nausea, vomiting, diarrhea, muscle pain, weakness, fatigue, and/or numbness or tingling in your hands and feet. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration of digoxin and colchicine may increase the plasma concentrations of both drugs. The proposed mechanism is competitive inhibition of P-glycoprotein (P-gp) efflux transporter in the intestine, renal proximal tubule and liver, resulting in increased drug absorption and decreased excretion. Rhabdomyolysis, which is a toxic effect of colchicine, has been reported.

MANAGEMENT: Caution is advised if digoxin and colchicine are used in combination. Serum digoxin levels should be monitored more closely whenever colchicine is added to or withdrawn from therapy, and the digoxin dosage adjusted as necessary. Patients should be advised to notify their physician if they experience signs and symptoms of digoxin toxicity (e.g., irregular heartbeats, slow pulse, nausea, anorexia, visual changes) or colchicine toxicity (e.g., abdominal pain, nausea, vomiting, diarrhea, fatigue, myalgia, asthenia, hyporeflexia, paresthesia, numbness).
