The proposed mechanism is competitive inhibition of P-glycoprotein (P-gp) efflux transporter in the intestine, renal proximal tubule and liver, resulting in increased drug absorption and decreased excretion. Rhabdomyolysis, which is a toxic effect of colchicine, has been reported.
MANAGEMENT: Caution is advised if digoxin and colchicine are used in combination. Serum digoxin levels should be monitored more closely whenever colchicine is added to or withdrawn from therapy, and the digoxin dosage adjusted as necessary. Patients should be advised to notify their physician if they experience signs and symptoms of digoxin toxicity (e.g., irregular heartbeats, slow pulse, nausea, anorexia, visual changes) or colchicine toxicity (e.g., abdominal pain, nausea, vomiting, diarrhea, fatigue, myalgia, asthenia, hyporeflexia, paresthesia, numbness).
