Talk to your doctor before using rilpivirine together with voriconazole. Combining these medications may increase the blood levels and effects of rilpivirine. At the same time, voriconazole levels may decrease, which may make the medication less effective. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if your condition changes or you experience increased side effects. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: The concomitant use of oral azole antifungal agents and supratherapeutic doses of rilpivirine (150 mg daily) may increase the plasma concentrations of rilpivirine and may modestly decrease the plasma concentrations of the antifungal. The mechanism is inhibition of CYP450 3A4-mediated metabolism by the antifungal agent and induction of metabolism by rilpivirine.

MANAGEMENT: Patients should be monitored for breakthrough fungal infections during concomitant therapy. A rilpivirine dose adjustment is not required.
